Hospice Care of Southwest Michigan can help when you need it most. We can help you – or someone you love – feel comfortable and safe. We begin by listening and answering your questions. We know that your experience is personal – so is our care. We’ll provide you with the physical, emotional and spiritual support you need to welcome each new day.

Time

When life expectancy is measured in months instead of years, there is still a lot of living to do. Hospice can help you do the things that are most important to you.

Your doctor may discuss hospice care with you. However, you can also call us to get more information on how hospice can help. We’ll even call your doctor for you, if you wish.

The final decision to accept hospice is yours alone, and you can change your mind and discontinue our services at any time.

Place

Given the choice, most of us want to spend our remaining time surrounded by familiar things and the people we love. For most of us, that means home. But Hospice Care of Southwest Michigan can care for you wherever you live, in your own home, at Rose Arbor or in another care facility. The choice is yours.

Wherever you call home, you will receive the dedicated attention and care of the whole hospice team. And we are available on-call 24 hours a day.

Team

Our highly trained hospice team works with you and your family to address all of your needs and make the most of your time together.

Your family doctor directs your medical care and treatment.

Your Hospice Medical Director supervises your care.

Registered Nurses are experts in pain and symptom control to keep you comfortable and alert.

Social Workers support you and your family and help coordinate community resources.

Chaplains provide spiritual support and work with your faith community.

Home Health Aides help with your personal care needs.

Trained volunteers offer companionship and relief to clients and their caregivers.

Grief Support Counselors provide support to family and friends for up to 13 months after a loved one’s death.
