Peter Dieleman, MD
Joined Hospice Care of Southwest Michigan as full time
Medical Director in December 2000. Dr. Dieleman is a
graduate of the University of Michigan Medical School, and
completed his internal medicine residency at Butterworth
Hospital in Grand Rapids and at the University of Michigan
Medical School. He is board certified in internal medicine
and practiced at the Community Medical Center for 10 years.
Dr. Dieleman has completed the American Medical
Association's Education for Physicians on End of Life Care (EPEC)
and
is certified as an EPEC trainer. Dr. Dieleman is board
certified by the American Board of Hospice and Palliative
Medicine.
As Medical Director, Dr. Dieleman is responsible for the
quality of patient care. He acts as a consulting physician
for hospice clients providing in-home visits for
assessment. Dr. Dieleman is available to confer with
community physicians and other health care professionals on
palliative care and pain and symptom management.

Michael
Raphelson, MD
Joined Hospice Care of Southwest Michigan in January 2002
and
serves as Medical Director for Hospice Care of Southwest
Michigan's divisions of Cass County Hospice and Hospice of
Van Buren County. Dr. Raphelson received his Bachelor of Arts in psychology from
Kalamazoo College and his Master of Science, graduating
magna cum laude in social work from the University of
Michigan School of Social Work. He received his medical
degree from Michigan State University, College of Human
Medicine. Dr. Raphelson completed his internship and
residency at Michigan State University/Kalamazoo Center for
Medical Studies. Dr. Raphelson
is
board certified in family practice and certified in Hospice
and Palliative Medicine.
