This is true with our social workers, as part of the team assigned to you at the start of your hospice experience. They’ll visit you at home, in a long term facility or a specialized care facility. They are there to listen, to find out your hopes, goals and concerns. They want to know what you need from them to be helpful for you.

Social workers listen and provide emotional support for you and your family to help navigate through this journey. They provide information on resources such as advance directives. They also provide information on resources within the community such as transportation, parking and food assistance. The social worker will help you achieve what is most important for you, such as having no pain or being awake and alert.
A social worker is the connector.
Your social worker can provide a communication connection between you and your family. During this tough emotional time, family members sometime think they know what’s best for you. They may not listen to your wishes. The social worker can be the intermediary. They also connect with your hospice team and convey your wishes.

You can expect compassion and caring for your needs. The social worker helps you and your family adjust and come to terms with being in hospice. But most importantly, they help you feel as good as you can for as long as you can, making your time before the end of your life as comfortable and meaningful as possible.
